Studies have revealed that the use of intra-articular pain pump catheters can cause Post-Arthroscopic Glenohumeral Chondrolysis (“PAGCL”). An intra-articular pain pump catheter is a tiny, flexible plastic tube that is implanted in the shoulder joint during surgery. The pain pump stays in place for several days after surgery to deliver pain medication (usually bupivacaine with epinephrine) to the shoulder joint. Pain management pumps have grown in popularity and are often used following arthroscopic shoulder surgery since they control pain without the side effects associated with narcotics. They also reduce the amount of breakthrough pain and minimize recovery time after surgery.

The most commonly used devices associated with the development of PAGCL include:

PAGCL destroys cartilage of the glenohumeral joint, the joint that connects the arm and shoulder. This condition occurs because the concentration of pain medication pumped directly into the shoulder is too high. PAGCL causes shoulder cartilage to deteriorate after arthroscopic surgery. Patients suffering from this condition experience severe pain and permanent impairment of shoulder functioning.

Symptoms include:

  • Shoulder stiffness;
  • Shoulder pain, both when in motion and at rest;
  • Shoulder clicking, grinding, and popping;
  • Shoulder weakness;
  • Decreased range of motion in shoulder; and
  • Narrowing joint space, visible on an x-ray.
